Caregiver Licenses

A caregiver’s designated patient(s) must have a completed Physician Recommendation Form (Adult Patient) (or Minor Patient), including the section where the qualified physician certifies the necessity of a caregiver.

Caregivers and patients must each sign a completed Caregiver Designation Form to establish the patient-caregiver relationship.

Caregiver licenses are valid until the expiration date for associated licensed patient(s), not exceeding two years. There is no application fee.

Caregiver applicants must provide proof of identity, proof of residency and an acceptable photo.

Medical Marijuana Reciprocity

Visiting out of state patients will have the option to obtain a short-term license to access medical marijuana when in Oklahoma. According to state law, residents living in another state with a valid MMJ license in their home state may apply for a temporary license to “buy, use, and grow” medical marijuana from an Oklahoma dispensary.

The out-of-state medical marijuana certificate and MMJ card will be valid for 30 days and must not exceed the expiration of the out-of-state MMJ card. Visitors can renew their Oklahoma MMJ license every 30 days, with a 1 week renewal period before the expiration.

Medical Marijuana Reciprocity

Caregivers

A caregiver is a person (a designated individual) who can pick up medical marijuana at a dispensary on behalf of a patient.  Each patient can designate up to two caregivers (they do not need to be a relative), and all minor patients must have at least one caregiver with a maximum of two (2).

Caregivers must submit a separate application for their medical marijuana caregiver card.

For caregivers to qualify  in Pennsylvania, caregivers must:

  • Be at least 21 and be a Pennsylvania resident.
  • Have a valid Pennsylvania driver’s license or a state-issued ID card (with current address).
